Job title: Project Manager, Coordinated Entry
Reports To: Director, Coordinated Entry, Outreach, and Diversion
Summary
Partners for HOME is looking to hire a self-motivated go-getter who will manage and implement a variety of projects for the organization. This position is excellent for someone who is flexible; able, and not afraid to take initiative; detail oriented; and looking to contribute to an organization that is at the forefront of making homelessness rare, brief, and nonrecurring in Atlanta.
Essential Job Duties and Functions
The project manager will be responsible for all aspects of work related to the chronically homeless population and the coordinated entry system. Duties are outlined below, and additional duties or responsibilities will be added when needed.
Chronically Homeless Population
Subpopulation Management
- Work with other Partners for HOME staff to create and manage the chronic By Name Lists (BNL) and serve as subject matter expert on the housing queue and issues related to chronically homeless population
- Report out on status of chronic BNL regularly and uses data from chronic BNL to guide navigation discussion/system needs
- Ensure relevant housing solutions are identified for everyone on chronic BNL and that movement toward housing is happening, utilizing resources outside of the Atlanta CE system when necessary
- Analyze trends of chronically homeless population to provide insight for staff and partners
- Case conference barriers for complex chronic individuals in system navigation to find resolution
- Manage housing queue vacancies for chronically homeless individuals
Strategy & Practice
- Oversee the work of ending Chronic homelessness in the city of Atlanta and continue to collaboratively define and refine the strategic vision based on real-time data, with thorough knowledge chronically homeless resources available and specific needs in Atlanta population
- Serve as expert in the latest best practices for engaging the chronically homeless and maintain up-to-date knowledge of rapidly changing techniques and innovations in the field.
- Coordinate trainings for provider community on best practices and techniques for engaging the chronically homeless in collaboration with SPM COC Training & Resources homeless
Coordinated Entry
HMIS and System Navigation
- Lead system navigation work, identifying system needs and barriers to housing using that information to guide decision making and navigation agendas
- Lead CE Assessor and CE Provider training on established in collaboration with SPM COC Training & Resources cadence
- Create CE training resources and materials based on workplan
- Coordinate 311 CE access points and develop 311 specific dashboards
- Provide information on the CES, including but not limited to accessibility to Coordinated Access Points
- Communicate issues and updates to community partners and stakeholders
- Refer clients to resources available outside of the CES
- Divert clients to the appropriate CES for their geographic location
- Monitor CE issues tickets in collaboration with the HMIS team
CE System Refinement
- Coordinate HMIS/CE system refinements such as the phased assessment approach and platform specific functionality
Housing Queue Management
- Maintain project specific knowledge for each RRH and PSH project in Coordinated Entry (for example, documents required for entry, diagnosis requirements, etc)
- Match clients to vacant RRH and PSH units within a maximum of 72 hours of unit becoming available
- Develop workflows to refine referral and matching processes
- Work closely with the HMIS team to handle HMIS tickets as they pertain to CES policy
- Create and maintain score cards and reports on housing queue and referral process to include statistics
Vacancy Monitoring
- Monitor housing vacancies including but not limited to: ensuring vacancies referred, enrolled and cleared correctly, all vacancies per project entered in timely manner
- Communicate and follow up on matches and referrals with agency case managers and/or assessors to ensure referrals are accepted within 72 hours by CM
- Follow up with providers regarding vacancy clean up and maintenance in timely manner
Inventory Management
- Manage voucher process with Atlanta Housing and other external entities for all voucher projects coming through Coordinated Entry
- Reduce amount of time from referral to enrollment
- Work with providers to manage their vacancy reports and handle any policy related issues
- Work with providers to maintain accurate vacancies in HMIS – incorporating information from contracts, grant agreements, HIC and GIW
- Work closely with HMIS to ensure that all new projects are set up completely and accurately, and that all room attributes reflect what is needed for matching.
Qualifications and Requirements
- Bachelor’s degree from an accredited four-year institution
- One year experience with direct client service (preference given to those with experience in homeless social service)
- One year experience in data management and reporting
- Experience working with homeless and housing issues
- Coordinated entry experience preferred
- Experience working in nonprofit environment
- Excellent writing skills
- Impeccable communication skills
- Acute attention to detail
Compensation
$52,800 to $59,400 annually with benefits.
